De Quervain’s (Thumb-Side Wrist Pain)

De Quervain's tenosynovitis irritates the two tendons that move the thumb, causing pain at the thumb side of the wrist. It is common in new parents and heavy phone users, and usually settles with splinting, load management and rehabilitation.

Understanding the condition

Small tendons, big wrist pain.

De Quervain’s tenosynovitis is irritation of the two tendons that run along the thumb side of the wrist and move the thumb. It causes pain at the base of the thumb and wrist, especially when gripping, lifting a baby or twisting the wrist.

It is common in new parents, in people who type or text a lot, and in anyone doing repetitive pinching or gripping. Hormonal changes around pregnancy and breastfeeding may also play a part.

Treatment usually combines a thumb-spica splint for a period, adjusting the tasks that irritate the tendons, and progressive rehabilitation. Injections can help persistent cases, and surgery is rarely needed. Numbness and tingling suggest a different problem such as carpal tunnel and need assessment.

Why it happens

The root causes we look for.

01
Repetitive thumb use
Texting, gaming, mouse use and pinching irritate the tendon sheath.
02
Lifting and carrying babies
The wrist takes sustained loaded thumb-and-wrist positions.
03
Hormonal changes
Pregnancy and breastfeeding are associated with higher rates.
04
Direct strain or injury
A blow or sudden overload can start it.
Symptoms we treat

Recognise any of these?

Pain on the thumb side of the wrist
Pain lifting a baby or gripping
Swelling or a tender lump near the wrist
Pain when texting or using a mouse
Catching or creaking when moving the thumb
Weak pinch grip
Our Approach

How we'll actually treat it.

STEP 1
1
Assess & test
Finkelstein's test and examination to confirm it and rule out other wrist problems.
STEP 2
2
Rest the tendons
A thumb splint and adjusting the tasks that irritate the tendons.
STEP 3
3
Rehab & mobility
Graded tendon loading and gliding exercises as pain settles.
STEP 4
4
Return to tasks
Technique changes for lifting, gripping and phone use to prevent recurrence.
Services Involved

The disciplines we'll combine.

Every programme braids two or three of our core services. Here's the typical pairing for this condition.

The results

Treatment that actually holds.

Settling the tendons and changing the way you grip, lift and use your phone is what prevents it coming straight back.

It is a wrist examination in which the thumb is tucked in and the wrist is bent toward the little finger. Sharp thumb-side pain supports the diagnosis.
A thumb splint for some weeks helps rest the tendons, usually alongside changes to how you use your hand.
No. De Quervain's is pain at the thumb side of the wrist, while carpal tunnel causes numbness and tingling in the fingers.
Rarely. Surgery is considered when conservative treatment and injection have not worked.
